Telehealth Consent

This page summarizes what you agree to when you start an online visit. The full consent is shown on the clinical platform (operated by HOO Longevity, our clinical platform partner) before any visit, and that version controls.

What telehealth is

A licensed provider evaluates you using information you submit online, and sometimes by phone or video. The provider may prescribe, decline to prescribe, or recommend in-person care.

What you agree to

• You are 18 or older and your information is accurate and complete.
• Telehealth has limits, and a provider may decide in-person care is more appropriate.
• The provider decides whether any treatment is appropriate, and no result is promised.
• Compounded medications are prepared for an individual patient and are not FDA-approved.
• You consent to communication through the secure patient portal and the contact details you provide.
• Awesentials provides no medical services.
